What this service is

The Palm Beach Metabolic Care portal and app let you record your progress between visits and stay in contact with your care team. They are tools for tracking and care coordination.

They are not a medical device, they do not diagnose, and they do not provide treatment recommendations. Nothing shown to you is a substitute for the judgement of your clinician.

This is not for emergencies

If you are experiencing severe symptoms, seek urgent medical care or call 911. Messages sent through this service are not monitored continuously and must never be used to report an emergency.

Your account

Keep your password to yourself. You are responsible for what happens under your account.

Tell us promptly if you believe someone else has access to it.

Accounts are for the person they were created for. Do not share one.

Accuracy of what you record

The information you enter is used by your care team. Record it accurately. If you notice a mistake in something you logged, correct it or tell us.

Availability

We aim to keep the service available, but it may be unavailable for maintenance or reasons outside our control. Do not rely on it for anything time-critical to your care.

We strive to keep the member portal and related services available, but we do not guarantee uninterrupted or error-free access. The portal may be unavailable from time to time for maintenance, updates, or reasons beyond our control. We disclaim any specific uptime or availability commitment.

Membership

Billing is handled through Nextech (the practice’s EHR / Practice Management system). The custom member portal itself does not process payments.

Membership in Palm Beach Metabolic Care is a monthly arrangement at the rate published on the website (currently $99 per month). Membership is billed monthly through the practice’s billing system (Nextech). There is no long-term contract. You may cancel at any time before the next billing date; there are no cancellation fees. Membership status is managed by practice staff; the member portal does not process payments. Prescription medications and individualized medical treatments, when clinically appropriate, are fulfilled separately by the patient’s pharmacy and are billed separately. The membership fee does not include the cost of medication and is not billed to insurance. Membership does not guarantee prescription approval, insurance coverage, or medication eligibility.

Ending access

You may stop using the service at any time and may ask us to delete your account. We may suspend access where it is being misused or where required for safety or legal reasons.

Governing law

These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Any dispute arising out of or relating to these Terms or the services shall be resolved exclusively in the state or federal courts located in Palm Beach County, Florida, and you consent to personal jurisdiction there. TO THE MAXIMUM EXTENT PERMITTED BY LAW, OUR TOTAL LIABILITY ARISING OUT OF OR RELATED TO THE MEMBER PORTAL OR THESE TERMS SHALL NOT EXCEED THE AMOUNT YOU PAID US FOR MEMBERSHIP IN THE TWELVE (12) MONTHS PRECEDING THE CLAIM. We disclaim all warranties not expressly stated in these Terms.
