This page describes what you are agreeing to when you begin care with Palm Beach Metabolic Care. It is an overview for you to read at your own pace. Your formal informed-consent document is reviewed with you and signed before treatment begins, and your clinician will answer any question you have before you sign anything.

The nature of physician-supervised metabolic care

Palm Beach Metabolic Care provides physician-supervised metabolic and weight-management care under the clinical direction of Plastic Surgery of Palm Beach, P.A. Care may include medical evaluation, laboratory work, prescription medication where clinically appropriate, nutrition and lifestyle guidance, and ongoing monitoring between visits through the member portal and app.

The portal and app are tools for tracking and communication. They are not a medical device, they do not diagnose, and they do not make treatment decisions. Every clinical decision is made by your clinician.

Risks, benefits, and alternatives

Every medical treatment carries potential risks as well as potential benefits, and alternatives — including lifestyle-only approaches, other medications, or no treatment — always exist. Before you begin, your clinician will discuss with you the specific risks, expected benefits, common side effects, and reasonable alternatives that apply to your situation and your medical history.

Prescription medications used in metabolic care can have side effects. Report anything concerning to your care team promptly, and seek urgent care or call 911 for severe symptoms.

Medications are prescribed only when clinically appropriate

Enrollment or membership does not entitle anyone to a prescription. Medications are prescribed only after a clinician has evaluated you and determined that treatment is clinically appropriate for you, and prescriptions continue only while that remains the case.

Online visits

Online visits are available to Florida residents only, and telehealth has limits: some situations require an in-person examination, and your clinician may require one before starting or continuing treatment.

Results vary

No outcome is guaranteed. Individual results vary with medical history, adherence, and many other factors, and no one at the practice will promise you a specific result.

Your right to ask questions and to withdraw

You may ask questions at any point, before or after consenting. You may decline any part of the recommended care, and you may withdraw from care at any time. Withdrawing does not affect your right to your records, described in our Privacy Policy and HIPAA Notice.

If you choose to stop a prescribed medication, tell your care team — some medications should be tapered or followed after stopping, and your clinician will advise you.

How your consent is documented

Your formal informed-consent document is presented to you, reviewed with you, and signed before treatment begins. A copy is kept in your clinical record, and you may request a copy at any time.
